Company Overview – Rooted Life
Rooted Life is seeking a Licensed Clinical Social Worker (LCSW) to provide direct, field-based case management and clinical support to a caseload of up to 60 clients with complex medical, behavioral health, and social needs. This is a fully onsite, community-based role requiring regular in-person engagement with clients in shelters, encampments, homes, and partner sites. The LCSW plays a critical role in delivering trauma-informed care, coordinating services, and supporting clients in achieving stability and improved health outcomes.

 

Key Responsibilities

Direct Client Care & Case Management

  • Manage a caseload of up to 60 clients providing health and housing focused case management 
  • Conduct comprehensive biopsychosocial assessments
  • Develop and implement individualized care plans aligned with client goals
  • Provide ongoing case management, including regular in-person visits
  • Deliver crisis intervention and stabilization support as needed

     

Clinical Support & Behavioral Health Integration

  • Provide clinical insight and guidance on behavioral health needs
  • Support clients with mental health and substance use challenges
  • Apply trauma-informed and harm-reduction approaches in all interactions
  • Coordinate with psychiatric and medical providers as appropriate

     

Care Coordination & Systems Navigation

  • Coordinate services across healthcare, housing, and social service systems
  • Facilitate referrals to community-based providers and track follow-through
  • Advocate for clients within healthcare and benefits systems
  • Support transitions of care (e.g., hospital discharge, housing placement)

 

Documentation & Compliance

  • Maintain timely, accurate documentation in the electronic health record (EHR)
  • Ensure compliance with Medi-Cal, CalAIM, and ECM program requirements
  • Complete assessments, care plans, and progress notes within required timelines
  • Participate in audits and quality assurance processes

 

Team Collaboration

  • Collaborate with Care Managers, Community Health Workers, and clinical staff
  • Participate in case conferences and multidisciplinary team meetings
  • Provide clinical consultation to non-licensed team members when appropriate
  • Supervision of Care Management team members

 

Qualifications

Required

  • Active Licensed Clinical Social Worker (LCSW) in the state of California
  • Master’s degree in Social Work (MSW) from an accredited program
  • Minimum 2–3 years of experience in case management, behavioral health, or community-based care
  • Experience working with high-acuity populations, including individuals experiencing homelessness
  • Strong knowledge of trauma-informed care and harm reduction practices
  • Ability to work onsite and in community-based settings daily

Preferred

  • Experience with Recuperative Care, Enhanced Care Management (ECM) and other CalAIM programs
  • Familiarity with Medi-Cal documentation and billing requirements
  • Experience using EHR systems (e.g., Ritten or similar platforms)
  • Bilingual (Spanish or other languages commonly spoken in the community)

This position operates in a fast-paced, mission-driven healthcare and housing services environment supporting staff who work directly with vulnerable populations. 

  • Fully onsite, field-based role
  • Daily travel within the service area required
  • Work conducted in community settings including shelters, encampments, clinics, and client residences
