As a key member of our medical team, you will play a vital role in delivering palliative care services, collaborating with multidisciplinary teams to develop effective treatment plans tailored to each patient's needs. This position emphasizes a holistic approach to care, aligning with our mission to enhance the quality of life for patients and their families.
Responsibilities
- Provide comprehensive palliative care assessments and recommendations
- Collaborate with patient families and healthcare teams for holistic treatment
- Develop individualized care plans that address physical, emotional, and spiritual needs
- Conduct regular patient evaluations and document care progress
- Communicate effectively with patients and their families to support decision-making
- Participate in interdisciplinary team meetings and case discussions
- Educate patients, families, and staff on palliative care principles
- Stay current with best practices in palliative care and related fields
Requirements
- MD or DO degree from an accredited institution
- Experience in palliative care, geriatrics, or internal medicine is preferred
- Valid medical license to practice in the state
- Strong clinical and diagnostic skills
- Excellent interpersonal and communication abilities
- Compassionate approach to patient care and understanding of patient needs
- A commitment to a team-based approach in providing care
- Ability to build rapport with patients and families effectively
Schedule: Monday-Friday, 8a-5p
Daily Volume: 10 admissions
Patient Population: Geriatric
Duties: Managing chronic conditions, preventative care & assessments, medication management, and care coordination
Support Staff: MA and RN
EMR: Cerner
Must be board-certified
